We are a certified Passport Acceptance Facility, authorized by the U.S. Department of State to accept and verify the documents, applicant signature and proof of identity materials required for passport applications.

WHAT YOU'LL NEED
BIRTH CERTIFICATE
It MUST be an original or certified copy and it MUST have the names of both parents of the applicant.
PHOTO ID
This is usually a Pennsylvania Driver's License or State Issued ID. If your photo ID was not issued in Pennsylvania, an additional form of identification is required.
PASSPORT PHOTO
You can have your passport photo taken at AAA, CVS, or Walmart. These locations charge a fee for passport photos. Note that the Hawley Library DOES NOT have the ability to take passport photos.
FEES
You'll need to pay the fees listed below in order for us to process your passport, so be sure to bring payment with you on the day of your appointment.
Check or money order made payable to the Department of State:
Passport Cards - Routine Service
Check or money order made payable to the Hawley Public Library, or cash:
$35.00 per passport or passport card. This is a State Department mandated fee that we are required to charge.
